Patient Information
We would like to thank you for making an appointment with our office. We are aware that each medical practice has different procedures, so having you become familiar with our policies will help us in our working relationship with you.
Appointments
Patients are seen by appointment only. Patients being seen as "scheduled work-ins" will see one of our providers as quickly as possible after patients with regularly scheduled appointments.
If you need to miss a scheduled appointment, please call at least 24 hours in advance. If you do not give 24 hours notice, there is a $50 fee for the missed appointment, due prior to your rescheduled appointment.

What to Bring to your Appointment
- A list of current prescription or over-the-counter medications you are taking, including dose and frequency
- Pertinent information about your medical and surgical history
- Any recent x-rays, CT's, MRI's, blood tests, or appropriate records you may have
- Note: If you have an HMO, POS, or Managed Choice policy requiring a referral from your primary care doctor, you are responsible for obtaining all referrals and making sure they are valid for every office visit.

Before your first appointment, you will need to fill out patient forms. You can download and print out our forms from our website and complete them at home. Please complete the printed forms and bring them to our office on your first visit.

TelepHone Calls
If you would like to know routine test results, please call our office one week after having the test to allow the facility enough time to get these results to us.
Our phones are turned over to the answering service at 4:30pm, Monday through Friday, as well as during the lunch hour 12:30pm - 1:30pm. If you have non-emergency questions for the doctor or nurse, please leave all messages on the doctor/nurse message line (option 3). Responses to these messages will be handled as soon as possible. For prescription refills, leave your message on the prescription line (option 3). |
Prescriptions & Refills
At least 24 hours notice is required to call in any drug refill. Please note, calls made on Friday may not be filled until Monday.
No additional requests for prescriptions or refills will be taken after 4:30pm, Monday through Friday, or on weekends. If you are in severe pain, go to the Emergency Room. You should check with your pharmacy after 5:00pm to see if your prescription has been called in. If you use a mail-order pharmacy, it is your responsibility to mail the written prescription to the pharmacy. We are not able to call in mail-order prescriptions. Prescriptions are written to last until your next scheduled office visit. No narcotic medications will be prescribed over the phone after office hours. If narcotic medications are needed after office hours, they will be dispensed through the emergency room. |
Insurance & Payment Info.
Lakeside Neurology, P.C., is a provider for Medicare and most major insurance plans.
We provide insurance billing. Anything not covered by insurance will be the responsibility of the patient. At the time of your visit, you will be responsible for paying the portion of your bill that your insurance plan does not cover. This includes the co-pay for each visit. Please become familiar with your insurance requirements. If your insurance turns down a claim because it is not a covered service under your plan, or because it is a pre-existing condition, etc., you are responsible for payment for these services. When necessary, our staff will work closely with patients who require a payment plan. If you have any questions regarding which insurance plans we accept or any patient billing concerns, please contact our office. Questions regarding your coverage and benefits should be directed to your employer or insurance company. |
